Our Commitment Home is the most patient-centered site to receive health care. The scope and variety of types of patients seen in home care demand strong clinicians with excellent communication, interpersonal, and assessment skills. Patients are now discharged much earlier from the hospital than in the past, and require highly skilled nursing care in their homes as they continue in their recovery from illness or injury. Our registered nurses are experienced home health practitioners who are skilled in the full range of home health nursing services. A nurse is available 24 hours per day, 7 days per week to assist our home care patients and their families, and new referrals as well. We are committed to providing holistic health care across the life span continuum for newborns, children, adults, and the elderly. Our plans of care are developed with each client and their family, setting goals that will achieve the most optimum level of wellness available for that individual.
