• Change lives, one person at a time, by providing a holistic approach to care in order to achieve optimal outcomes for each person we serve.
• Provide all levels of home care service in collaboration with the physician, in order to minimize the effects of illness, accident, or disability.
• Provide a broad range of cost-competitive, quality services with patient need/demand governing the scope, allowing patients to be involved in establishing, implementing, and evaluating services.
• Maintain persons in their homes as long as possible and provide services in the least restrictive setting, recognizing that individuals and their families have a right to self determination, participating in their plan of care through informed decision making.
• Provide comprehensive health education as an integral part of our service outreach.
• Assist individuals affected by chronic illness or disability to adapt in a positive and constructive fashion in order to retain or regain their maximum participation in society and in their own self care management.
• Strive to achieve the rehabilitation of aged, diseased, and/or disabled individuals within the structure of their home environments, allowing maximum contribution, independence, and retention of self respect.
• Function as a liaison between the persons we serve and the community at large in order to assist them in achieving the most optimal level of wellness and function possible.
• Meet the health care needs of the community in an innovative and creative fashion, working along side other health and welfare agencies to promote community awareness of health and wellness issues, disease management, and illness/accident prevention.
• Provide leadership in the community to mobilize efforts to coordinate health services for persons in need and facilitate their access to those services.
• Ensure that each member of our staff possesses and demonstrates the knowledge, understanding, attitudes, and behaviors necessary to effectively and efficiently serve each of our home care clients.
• Support the dignity and self respect of individuals facing the end of life by providing sensitive and appropriate care and coordination of services to meet the special needs of the dying patient and their loved ones/caregivers.
