Intermittent Skilled Nursing Care
A skilled nursing service is a service that must be provided by a registered nurse, or a licensed practical nurse under the supervision of a registered nurse, to be safe and effective. Under the plan of care, a registered nurse makes home visits to provide specific nursing care (interventions) that cannot be performed safely and effectively by a family member or other caregiver. In addition, when the patient needs skilled care and there is no one able or willing to be trained to provide the care, the services of a home health nurse are reasonable and necessary. The following is a list of some of the services the home health nurse provides:
- Skilled observation and assessment of a patient’s condition for a period of time due to a reasonable potential of a complication, further acute episode, or current unstable condition.
- Management and evaluation of a patient care plan where underlying conditions or complications dictate that only a registered nurse can determine if essential nonskilled care is being performed effectively by caregivers in order to promote recovery and ensure medical safety.
- Teaching and training activities
necessary to teach a patient, his family or caregivers how to manage his treatment regimen with proper medication administration and correct technique for various treatment procedures.
- Administration of medications by injection
- I.V. infusion therapy (of various medications including anti-infective agents, chemotherapy)
- Medication regimen management for those with new and/or complex medication treatment programs including oral medications, who require teaching to safely manage their own care.
- Ventilator management and tracheostomy care
- Gastrointestinal tube feedings (post-op or if complications)
- Ostomy management and care (post-op or if complications)
- Foley catheter management and care
(sterile insertion, irrigation, replacement of)
- Venipuncture to obtain blood specimens
(for necessary lab work to treat illness or injury)
- Wound Care and Management
- Rehabilitative Nursing care for those with neurological and musculoskeletal disorders such as stroke, bone fractures, joint replacements, amputations
