Registered Nurse Consultations/Assessments

Referrals are received from a variety of sources requesting an RN assessment of an individual’s health status and consultation regarding current health care needs. Click on this section to learn about this service.

Individualized Plans of Care

Each home care client has an individualized plan of care developed by the nurse with input from the client and family. Specific goals are agreed upon to promote recovery and achievement of the best level of independent function and self care.

Intermittent Skilled Nursing Visits

Medicare, Medicaid, and private insurance companies pay for scheduled nursing visits to the home to provide skilled nursing services necessary for the continued recovery from illness or injury and the promotion of effective management of the client’s treatment regimen.

Rehabilitative Therapy

The skills of a physical therapist may be required for those clients who are recovering from certain illnesses or injuries and are in a weakened condition. Some examples would be those individuals who have suffered a stroke, fractured bones, amputations, joint replacements, or who suffer from certain neuromuscular diseases.

Home Health Aide

Under Medicare, scheduled home health aide visits are provided for personal care while a client recovers from an illness or injury.

Skilled Nursing/Respite Care

Under certain Medicaid Home and Community Based Waiver Programs, eligible clients who require ongoing, skilled nursing care may receive long term nursing services in the home rather than going to or staying in a nursing home facility.

Private Duty Services

Skilled and unskilled nursing services are provided in the home on an hourly basis to clients who desire to pay for these services directly. Private duty nurses and nurse aides are available to provide care in the home according to the individual’s needs.

Insurance Assessments/Pre-Certifications

At the request of an insurance company, one of our registered nurses will perform an independent health assessment in an individual’s home.
