Rehabilitative Therapy
Some patients require the skills of a physical therapist due to a weakened condition with poor endurance, decreased mobility, and balance and gait issues. Often physical therapy is part of the overall plan of care that also includes skilled nursing services.

The therapist assesses the patient’s need for specific rehabilitation services such therapeutic exercise, gait training, range of motion exercises with the goal of improving strength, balance, endurance, and ultimately mobility and independent function. A physical therapy program is developed and then implemented in the home by the physical therapist. In addition, the therapist would assess the need for any special equipment such as a wheelchair or other mobility and/or functional enhancing aids. That equipment is then obtained and the patient and family trained in its proper and safe use.

The original referral most often comes from physician office or hospital discharge planner with a physician’s order for physical therapy evaluation and treatment. The therapist’s plan of care for an in-home therapy program is submitted to the physician for review and signature once the initial home visit and assessment is completed.
